Hello,
Many new patients have challenges with finding the office location, or other issues, so please read everything below to make this more seamless for you and less of a worry. 🙂
New Patient Paperwork

Please complete and bring them to your appointment. (If you can't print them, please let us know and arrive 15 minutes early to fill them out.)
Directions
The office is located at 603 Hemlock St, Brookings, OR 97415 (Google Maps link).
(Picture #1): Enter from Hemlock St. on the ground floor (North/east side of the building), take the elevator to the 3rd floor.
(Picture #2): Step off elevator and directly across into the waiting room. (Picture #3)
(Picture #4): Someone will bring you into the shared office inside Visana Wellness at your appointment time.
Payment & Insurance Info
The typical cost of this first visit (or returning visit if you haven't been in for two or more years) is $160 ($156 for Medicare). Dr. Rob is not currently accepting personal injury (PI), auto accident (auto), or workers' compensation (WC) cases which require additional care and time.

If you require follow-up care, treatment will be $45 ($41 for Medicare), as long as you are seen within 2 years, don't have a significant new injury or condition, and are not PI, auto, nor WC.

Payment can be made at the time of service by cash, check, credit/debit card, PayPal/Venmo or TapToPay.
Insurance will not be billed unless you have Medicare, then billing will be submitted to them for your reimbursement. For all other insurances, we can provide a detailed receipt ("Superbill") with the information necessary for you to submit to your insurance directly for any reimbursement they will give you.
Appointment Reminders + Check-in
You will receive a reminder call, text or email with a link to confirm your appointment a day or two prior to it.
Please reply to the automated message to confirm, cancel or reschedule your appointment.
You will not be able to use the check-in reminder the day of your appointment until after your 1st visit and we update your date of birth. We do not ask for this here or via text, as they are not HIPAA-compliant for Protected Health Information. If communication requires HIPAA-compliance, we have other encryption or a different email address, or can communicate through our office software, once we have your new patient paperwork.
The automated appointment reminders only receive the specific replies they list, 1,2 , or 3, and not any other text message or email response.Â
