CAPT Directory Form
Submit this form to keep up with the latest CAPT information! By opting in you'll receive email reminders about CAPT meetings and events like Homecoming, Dine Out, Parents Night, Raise Craze, The Holiday Shop, Teacher Appreciation, Spirit Wear, and so much more! Opting in does not mean you have to attend CAPT meetings or help with CAPT events, it is just another way for you to stay informed. If you opt in and later decide you want to opt out, please email jfkcapt@gmail.com 

Room Parent Form (2023-2024)
Each classroom benefits from 2 room parents each year. Being a room parent is an incredible way to stay connected to your child’s classroom activities, the other parents, and the teacher!

CORI Form
CORI is a name-based criminal record check. If you are interested in helping at the school in any capacity - whether it’s for the book fair or helping at a classroom party, you MUST complete a CORI form with the JFK prior to the occasion. In addition to filling out the form, you will also need a copy of your driver’s license. The CORI check takes a few weeks to process, so do it as soon as you can, and then know it’s good for 3 years! 

Please submit your completed CORI form and a photocopy of the front and back of your driver's license via email to Alana Marino, JFK front office, at marinoa@cantonma.org.

Fingerprinting
Fingerprinting is a standard, mandatory background check for all parents/guardians who would like to chaperone a field trip. The process is completed at another site (NOT at the JFK or the administration building). More information about the fingerprinting process can be found here.
