Jefferson County Care Transitions Coalition

Mission: To improve the quality of care for health care consumers who transition among health care settings.

Vision: The Jefferson County Care Transitions Coalition envisions the transition of patients/residents between health care settings and pracitioners throughout Wisconsin will be well-coordinated between all institutions, pracitioners, and community service organizations with the patient and caregiver as the center of care.

Purpose

- To collaborate and cooperate to prevent duplication of effort and resources

- To share and exchange data and analytics to drive the allocation of resources

- To collaborate and encourage efforts and best practices of organizations with shared visions

- To promote effective strategies to improve and ensure appropriate transitions of care

- To advance policies that further the vision

Meetings are held at the Jefferson County Aging and Disability Resource Center (ADRC) the second Thursday of every month from 12:30 - 2:00 pm.
