Using Google Forms to manage
In Google Drive, select NEW and then Google Forms.
To add a banner at the top of the page, select the palette icon, and then upload the graphic to use as the top banner.
To add a graphic to a question, add a new question, then click on the image icon to upload an image from your computer, or search for an image in Google.
To add text a) type in the top line of the question box, or b) use the 3 dots at the top left of the question box and select 'Add a caption'. (The image cannot be selected or the 3 dots will not appear.)
The Google Form Template (link below) contains 14 different question formats. To create your own Google form, make a copy of the Template and rename it. Use this copy to edit the form. Delete any of the question formats you do not want to use. If you want multiples of one question format, such as True/False, use the duplicate icon to copy as many as you need.
NOTE: The Google form Template contains the following question formats. (The link will force you to make a copy of the form.)

Beginning / End - multiple choice
Beginning / Middle / End - multiple choice

Use the Share option to share a link with your students, or email the link to them. Once they have completed the form, be sure they remember to SUBMIT the form.
To see their answers, click on the 'RESPONSES' tab at the top of the form. This will let you create a spreadsheet of the answers. The spreadsheet can be sorted by any column (class, name, etc.) (Hint: decrease the width of all the answer columns to be able to see more answers at once. AND, freeze the top row and side columns so you can see the names as you scroll across and down the page.)
To share a Google form with another teachers so they can use it, copy the URL link at the top of the browser and paste into an email, etc. To force them to make a copy, change the word 'edit' near the end of the link to the word 'copy.'
Mote - The Chrome extension that lets you easily add voice comments and audio content to shared documents, assignments, emails and forms. Mote is integrated into Google Docs, Slides, Sheets, Forms and Classroom - and Gmail - for easy recording and playback.
