As older adults with advanced illness increasingly rely on the support of others to help manage their health, there is a critical need to foster approaches for effective integration of the family and other caregivers in mobile health.
We are developing and test the Social Convoy Palliative Care (Convoy-Pal) mobile application. Convoy-Pal is a novel intervention integrating the family and other caregivers in digitally-supported palliative care to promote symptom management and quality of life.

Heart failure (HF) disproportionately affects older adults and results in high hospitalization, symptoms, and care-partner burden.
The HF Symptom Tracker was designed to help patients monitor symptoms and share symptom information with their care-partners. Older adults found the app acceptable to use.
