HARDSHIP APPLICATION GUIDELINES
The Hardship Fund assists members of this Local Union with a proven hardship covered by these guidelines. The Hardship Committee (President, Financial Secretary, Trustees) approves or disapproves spending based on these guidelines.
1. Member must be in good standing for 12 consecutive months and current on dues.
2. Hardship defined as fire, injury, or lengthy illness causing 3+ consecutive weeks missed in one month. Request must be received within 30 days of hardship or 2 weeks after returning to work.
3. Maximum payout: $400 at one time, $800 per year. Two applications per year. Disaster loss may allow full $800 at once.
4. COVID/FLU only approved if hospitalized for 2+ weeks. Quarantine does NOT qualify.
5. Provide current address and phone number. Incorrect info = denial.
6. Documentation from doctors required with every application. Must state days missed from work under doctors’ orders.

SUBMISSION OF DOCUMENTS
You MUST submit documentation from your doctor with each hardship application. If documentation is not submitted, your application WILL BE denied. Documents from doctor MUST show last day worked and return to work date. If you do not have access to a printer you can open the hardship application then save it on your device. Go to your saved location open the application and it will be fillable. Once you have the application filled out completly save it then uploaded it with the link above make sure to also include your documentation.
