It’s all about putting those awesome life skills to use and working towards the goals in the Transition Plan. Luckily, there are fantastic programs to help out for those who qualify! 🌟 These include Extended Foster Care (EFC), Post Secondary Services and Support (PESS), and Aftercare Services.
