We are calling the attention of all active beneficiaries of the College Scholarship Program to fill out the Google Form Link. This form is intended for creating the Iskolar ng Bataan ID Card.

Please check below the list of names and note your ISKOLAR ID NUMBER, as this will be required when filling out the google form. Please also change the FILE NAME of your 2x2 picture and use your ISKOLAR ID NUMBER.
