This National Center for Equitable Care for Elders learning collaborative will explore the role of health centers in meeting the complex and evolving needs of community-dwelling adults. Participants will discuss innovative approaches to age-friendly care delivery and cross-sector partnerships that can effectively support this growing patient population.
After participating in this learning collaborative series, participants will be able to:
Describe the 4Ms of an Age-Friendly Health System: What Matters, Medication, Mentation, and Mobility
Apply key principles of person-centered geriatric care to current health center processes and procedures
Consider shared strategies for expanding older adult-focused programming and partnering with local organizations to support patients to safely age in place

NCECE Learning Collaborative Community Expectations:
Active participation in NCECE interactive learning collaboratives is essential to the productivity and success of these peer learning opportunities. Our team of NCECE faculty and subject matter experts facilitate these peer-to-peer sessions to provide health centers with the fundamental knowledge and training to better serve their older adult patients. To ensure a vibrant and dynamic learning environment, participants are expected to adhere to the following guidelines:
Arrive prepared to each session, having completed any pre-assigned readings or tasks.
Attend all scheduled meetings and sessions and communicate promptly if unable to attend.
Actively contribute to discussions through sharing insights, experiences, asking questions, and offering feedback.
Apply insights and knowledge gained from the collaborative to real-world situations.
Provide feedback (evaluation) post-session to cultivate improvement for future learning collaboratives.
By adhering to these expectations, participants can have a fruitful, productive, and supportive learning environment that maximizes the benefits for everyone involved.
Andrea Wershof Schwartz, MD MPH AGSF is co-director of the Aging and End of Life Theme at Harvard Medical School and is a national leader in the Age Friendly Health Systems initiative. Dr. Schwartz is the Associate Director for Clinical Innovation in the New England Geriatrics Research Education and Clinical Center at VA Boston and Associate Director of the Harvard Multicampus Geriatrics Fellowship.
Arielle Mather, MPH serves as Senior Program Manager at NCECE, overseeing the planning and delivery of the Center’s trainings and technical assistance activities. She has spent many years in the Boston nonprofit sector addressing the needs of older and vulnerable populations, as well as the providers who care for them.
