Illuminate providers are available for individual, family or couples psychotherapy. Illuminate providers are available to work with individuals of various ages. Call or e-mail to see if a provider is available to fit your needs.

Contact Angela at 330-227-1716 or Abby 330-227-1713 to schedule your intake. Business contact hours are 9AM-5PM, Monday-Friday. Individual appointment times may vary based on individual need and therapist schedule availability. 

Payment

Full payment for session is due at the time of services. Illuminate Mental Health accepts payment in the form of check, credit card, or cash. Illuminate Mental Health utilizes their Electronic Health Records system to process credit card payments. Full Billing Rights and Duties information will be provided prior to the first session. 

Illuminate Mental Health utilizes a sliding fee scale for individuals who do not have insurance. Spaces are limited. Contact a provider for details. 

Limitations

Illuminate Mental Health, LLC and their providers are not licensed to treat Substance Abuse Disorders. A referral to appropriate services will be made if substance abuse impedes treatment at Illuminate Mental Health, LLC.

Illuminate Mental Health, LLC does not offer emergency or crisis services. If you are in need of emergency or crisis services, please call the National Suicide Prevention Hotline (800-273-8255) or go to your local emergency room.

Intake Paperwork

Intake paperwork is sent via an online portal. An Illuminate representative will need a valid e-mail address, the client's first and last name and date of birth in order to send the portal information. If you are unable to complete the intake paperwork electronically, please e-mail a representative directly to request an alternative method of completing the paperwork.
