Patient contact and communication
· Re-confirm and verify demographic, contact and insurance information
· Interview patient as to circumstances and facts surrounding injury, illness and treatment
· Execute Authorized Representative form
· Execute NFAOB (for no-fault)
· Identify and complete any special forms required by insurer
· Cooperate in filing late payment claim with appropriate administrative agency
· Contact benefits coordinator at Employer (if sponsored plan)
· Is the patient continuing to receive treatment for the same injury/illness?
· Prepare letter for patient’s signature-providing additional documentation and/or information
· Keep the patient fully informed throughout the process
· Assist in contacting other providers for information or documentation
Educate the patient as to process and procedure
