Request for Funds

Use this form when requesting money from the Athletic General Fund, IAB or Sport Sub Account

IF you are requesting from the Athletic General Fund or IAB you need to fill out form and email it to the Athletic Director for approval. They will then present the request in an IAB monthly meeting. 

IF you are requesting funds from a Sub-Account for a specific sport you need to fill out the form, coach must sign and then you can submit to the Treasure with supporting documentation.
