By submitting your story, you're sharing it with the I CHECKED THE BOX PROJECT for consideration in storytelling, educational material, social-media, advocacy, and book projects.

SUBMITTING A STORY DOES NOT GUARANTEE PUBLICATION.

You will have the opportunity in the submission form to tell me how you are comfortable with your story being used, including whether you want to remain anonymous.

Please don't include identifying information about other people - particularly children, patients, clients, students, or healthcare providers - unless you have appropriate permission to do so.
