Note to Parents--An athlete's eligibility is dependant on each of these forms being completed and submitted on DRAGONFLYMAX; or turned in to Coach Harris or Coach Webb.

1. Create a DRAGONFLY MAX Account

2. Upload Physical Form to Dragonfly

3. Upload Birth Certificate to Dragonfly

4. Electronically Sign Concussion Form Online on Dragonfly (or Print Concussion Form & Turn In)

5. Electronically Sign Consent Form Online on Dragonfly (or Print Consent Form & Turn In)

6. HCS Permission Waiver

7. Out of County-Overnight Trip

8. TOC Health Permission Form

9. 4th Pd Release and Transportation Release

10. Drug and Alcohol Policy

11. NFHS Sportsmanship -  To be completed by ALL New Athletes Only. Must be the High School Version.
