OPTION 1: Paper Order Request is available at the Humphreys Middle School Main Office. Please fill the form entirely with payment (cash), if ordering with PayPal, place it under "Form of Payment" on the form.
OPTION 2: Fill out the HMS Spirit Wear Form below.
When making HMS Spirit Wear Order below, your product(s) will not filled until payment has been received (Cash or PayPal).
Cash payment is accepted at Humphreys Middle School Front Office. Envelopes are available for submission. Place envelope inside PTSO box with Your Name and Child's Name.

T-Shirts/Hoodies - Will be unixsex, cotton, Korean Brand - Similar US size listed on forms (paper and below).
SUGGESTION ON SIZING: Order a size bigger to allow for shrinkage and growth of student.
