Humble Heart Transport Company provides non-emergency medical transport. All trips require AHCCCS-eligible clients, with Prior Authorization for each trip over 100 miles. If in need of transport, feel free to contact us!

Our Home Care Attendants are expected to fullfill:
- Attendant Care
- Traditional Care
- Agency with Choice
- Personal Care
- Hygiene (grooming)
- Bed/Walking Transfer
- Feeding Assistance
- Bladder/Bowl Incontinence Aid
- Home Maker
- Sweep, mop, dust, cook, wash dishes, laundry, etc.
- Respite Care
- Limit 600 hours per contract year

- Medical Appointments
- Children's Dental
- Women's Health
- Physical/Occupational Therapy
- Chemotherapy/Radiation Treatments
- Dialysis Treatments
- Rehabilitation AA Centers (w/ Referrals/Service plan)
- Nursing Home
- Member Placements
- Transfers Hospital Placements
