Please upload the TB test result (X-ray exam only) on the Google form as well.
X-ray examination is only acceptable.
The result should be issued after March, 2024.
If you need a health form to get a TB result, please use our health form.
You will need a Google Account to do this. If you do not have a Google account, you can send them to summer@hufs.ac.kr by email.
