MEMBERSHIP

Attend the PMES (Monday–Friday, 10:00 AM–3:00 PM) at the office. 

Fill out the membership application form and submit the following requirements: two (2) 1x1 or 2x2 ID photos, Certificate of Employment (if employed), business permit (if you have a business), and marriage contract (if married).

Pay ₱1,500 w/o insurance or ₱3,000 w/ insurance

For ₱1,500 pesos = membership fee - 100, processing fee - 100, ID - 100, passbook - 100, share capital - 1,000, and savings deposit - 100. 

For ₱3,000 pesos = membership fee - 100, processing fee - 100, ID - 100, passbook - 100, share capital - 1,000, savings deposit - 100 and healthcare - 500 and gyrt - 1,000.
