Our management is available to you 24 hours a day to ensure that all of your needs are satisfied and that all of your concerns are addressed. Our bundle of General Services includes support with all aspects of Daily Living, Care Management, Transportation, and Specialty Services.

Daily Activities

• Bathing and Dressing Assistance: Every day, we assist you in getting dressed and ready for the day.

• Assistance with Walking and Transfers: We support you with your walks and transfers by utilizing assistive equipment and the right technique to avoid falls.

• Medication Management: We manage your medication schedule for you.

• Toilet Transfer Assistance: We assist you with toilet transfers to ensure your safety in the bathroom.

• Personal Hygiene: Shaving, oral hygiene, and hair care are all areas in which we may aid.

• Meal Planning and Preparation: We will plan and prepare nutritious meals for you on a daily basis.

• Housekeeping and Laundry: Our light housekeeping and laundry services will assist you in maintaining a clean and tidy home.

• Care and Companionship: Our caregivers offer companionship and social activities.

• Safety and Supervision: We detect and handle potential safety hazards in your house, ensuring that it is a safe place for you to live.

Supervision of Care Management

• The caregiver and his or her family: Each new caregiver is introduced so that you can get to know them. Just like you, we don't want strangers caring for you.

• Family Liaison for Client Care Plan: Every aspect of your care will be discussed and communicated to your family.

• Constant Monitoring of Safety and Care Procedures: Safety and care procedures are regularly monitored.

• Monitoring for Changes in Condition: If your condition changes and different forms of treatment are required, we will make the necessary changes.

• Re-Hospitalization Prevention: If you have recently been hospitalized, we will collaborate with your physicians to ensure that all follow-up instructions are followed.

• Blood Pressure Monitoring: Your blood pressure will be checked on a regular basis and any changes reported to your physician.

• Medication Coordination: The prescribing physician and the pharmacy will work together to coordinate medication dosages and refills.

• Oversight of Diabetes Care: Insulin levels are tested on a regular basis.

• Hospice Care Coordination: If hospice care is required, we will coordinate the transfer and ensure that all necessary arrangements are made.

Transportation

We will transport you to locations within your immediate vicinity.

• Appointments with Physicians: Transportation to and from physician appointments.

• Grocery Shopping: We will shop for you!

• Social Activities: If wheelchair transportation is required, we will accompany you to social activities in your local area in your car or in our wheelchair van.
