STUDENT CONTRACT and PARENT CONSENT
DUTIES AND RESPONSIBILITIES
I commit to respecting the conditions listed below during the in-school activities (Pre-Placement and Integration) and during my time at the Cooperative Education placement:
I will be punctual and assiduous.
I will take responsibility for my actions and my behaviour.
I will wear appropriate clothing and have a clean and well-groomed appearance.
I will be kind and polite towards my supervisors and my work colleagues.
I will always warn my placement supervisor and teacher, either in advance or at least very early the same day, if I need to be absent from work at the placement or school.
If applicable, I can attend my usual courses every day at school.
I will let my teacher know when my placement has a problem.
I will immediately notify my placement supervisor and teacher in case of injury.
I will regularly submit my Co-Op log sheets according to the established process in my school. My Log Sheets will be completed with care.
I will be honest and hard-working.
I will diligently follow all instructions concerning health and safety in the workplace.
I will attend all Pre-Placement sessions at school.
I will attend all placement integration sessions at school and warn my placement supervisor in advance of my absence during those days.
I will hand-in the Rich Summative Task (RST) project to my teacher. I will follow the instructions I've been given.
I have read and accept the conditions mentioned above. In case of non-compliance, I know that my participation in the Co-Op program will be re-evaluated and that my placement could be cancelled, resulting in the loss of one or more credits.
INSURANCE
It is understood that:
The student must be covered under OHIP (Ontario Health Insurance Plan) and carry his or her OHIP card;
The School Board’s liability insurance covers the student who is registered in one of the school’s programs, including co-op (unless the student is paid while attending Co-op; then, the insurance will be covered by the employer);
The student and parent have taken note of the accident insurance plan that is available in September through the school;
the student is also covered by the Workers’ Safety and Insurance Board of Ontario (WSIB) through the Ontario Ministry of Education while performing Co-Op duties/functions at the placement (unless the student is paid while attending co-op; then, WSIB will be covered by the employer).
