• Grade 12 Copy of Grades (1st grading period only)

• PSA/NSO Birth Certificate

Requirements for Enrollment

• Original and photocopy Grade 12 Report Card

• Photocopy of PSA/NSO Birth Certificate

• Original Copy of Certificate of Good Moral Character from Senior High School

• Undertaking signed by the student and parent/guardian [please download here]

• One (1) copy of 2x2 latest photo

• Photocopy of Marriage Certificate (for married female applicants only)

*Additional Requirements for Filipino Applicants from Foreign Schools

• For Philippine schools abroad (any grade level): Permanent Transcript of records with LRN (Learners Reference Number

• For Foreign schools abroad: Apostilled Permanent Transcript of Records and Diploma from originating school.

*For delayed submission of the Grade 12 Report Card, applicants may proceed with the enrollment, provided that they sign a “Promissory Note for Conditional Enrollment” upon enrollment.

2. Original Copy of Certified True Copy of Grades

3. Photocopy of PSA/NSO Birth Certificate

4. Original Copy of Certificate of Good Moral Character from last school attended

5. Undertaking signed by the student and parent/guardian [please download here]

6. Original Copy of Barangay Clearance

7. One (1) copy of 2x2 latest photo

8. Photocopy of Marriage Certificate (for married female applicants only)
