Membership Forms

2026-2027 MEMBERSHIP PACKETS ARE DUE SATURDAY SEPTEMBER 12 2026 BY 5 PM

Mail to: HHSRA Maui District, P.O. Box 1587, Makawao HI 96768

Drop off: Po'okela Arena, white drop box at concession stand

Jr High and High School contestants:

• register on the NHSRA portal and pay fees

• important! Minors Release must be notarized for contestant and both parents

• upload all forms to your NHSRA portal AND turn in hard copies

  • 1 - membership form signed (no notary necessary)

  • 2 - notarized minors release

  • 3 - social media waiver 

  • 4 - grade and conduct from OR homeschool verification + DOE form 4140

  • 5 - current report card (4Q of last school year at this point in the year)

Keiki contestants:

• print packets and turn in hard copies only

• pay by cash or check made out to HHSRA Maui District (keiki) or K Ranch Roping (leadline)
