• 4 Passport Size Photo White Back Ground Ear Should be Visible

• Birth Certificate 

• Father Passport 

• Mother Passport

• 1 residence proof ( If Owned House then any 1 utility bill, Name of either parents is must on the copy of utility bill Eg - Electricity bill , Gas Bill , Water Bill, MTNL bill or Bank Passbook Photo attached and with stamp of bank authority )

                          Or 

• 1 residence proof ( if Rented House then rental agreement and Owner Noc)
