Family planning & goal setting- This process helps the individual/family identify strengths, areas of concern and support systems. Goals are established by the family and a plan is created.
Peer Support/Coaching- Referred to as Navigators and Family Support Specialists, participants are connected to resources, assisted with accomplishing goals and completing plans, assisted with any systemic requirements (court, probation, DHS, Juvenile Justice), and provided emotional support. These services are provided by trained individuals with experience overcoming barriers.
Provider & System coordination- Contacting Social Workers, Case Managers, Probation Officers, etc. to advocate for our participants and to promote collaboration between providers in an effort to address concerns, problems and progress from a "team" approach. This is important to help reduce overwhelm participants may feel from being "pulled" in different directions from various providers.
Virtual Services- Online workshops, trainings, groups, and/or forums will be proveded to participants as part of services. Some of these services can be offered to providers for a fee.
