I have been wearing Lively hearing aids, and recently inherited a pair of Oticon OPN1 hearing aids that had never been used. I mailed them to HearSource to reprogram to my specific audiogram. Analog hearing aids (think of your parents or grandparents generations) were adjusted using tiny screwdrivers to manipulate volume, low pitch (bass) and high pitch (treble) sounds. Hearing aids of today are complete digital technology- essentially tiny little computers designed specifically for your ears. Digital technology allows for hundreds of parameters to be adjusted and focused specifically towards your listening and communication needs. We program hearing aids based on hearing loss in front of the patient, with special emphasis on lifestyle and personal listening preferences. It is necessary to make several appointments for adjustments to the hearing aids after the hearing aids have been worn for a period of time as your brain is continuously adapting to the new sounds.

Speech Mapping: Speech mapping is a form of REMs that allow us to identify the frequency range and volume of speech sounds reaching the eardrum. Using speech mapping, we are able to track the digital components of hearing aids to ensure they are working accurately and continue to deliver a quality speech signal.

A surround sound system is used in order to simulate real noises and what it will be like for the user to make use of the hearing aid in a real-world scenario. In our daily lives, we find ourselves in situations where there are many different noises going on around us, and by simulating this, the hearing aids can be programmed to deal with that.

Troubleshooting is a major part of this process too. Faults can be found and things can be changed so that the user experiences the use of their hearing aid in the real world in the way they should. It prevents problems from arising because it will all be programmed correctly from day one of real-world usage.

Objective:  Voice-aligned compression (VAC) is a method used in Oticon's hearing aids to provide more comfortable hearing without sacrificing speech discrimination. The complex, non-linear compression curve for the VAC strategy is designed based on the frequency profile of certain spoken Western languages. We hypothesized that hearing aids could be further customized for Japanese-speaking users by modifying the compression curve using the frequency profile of spoken Japanese.

Methods:  A double-blind randomized controlled crossover study was performed to determine whether or not Oticon's modified amplification strategy (VAC-J) provides subjectively preferable hearing aids for Japanese-speaking hearing aid users compared to the same company's original amplification strategy (VAC). The participants were randomized to two groups. The VAC-first group received a pair of hearing aids programmed using the VAC strategy and wore them for three weeks, and then received a pair of hearing aids programmed using VAC-J strategy and wore them for three weeks. The VAC-J-first group underwent the same study, but they received hearing aids in the reverse sequence. A Speech, Spatial and Qualities (SSQ) questionnaire was administered before beginning to use the hearing aids, at the end of using the first pair of hearing aids, and at the end of using the second pair of hearing aids.

Results:  Twenty-five participants that met the inclusion/exclusion criteria from January 1 to October 31, 2016, were randomized to two groups. Twenty-two participants completed the study. There were no statistically significant differences in the increment of SSQ scores between the participants when using the VAC- or the VAC-J-programmed hearing aids. However, participants preferred the VAC-J strategy to the VAC strategy at the end of the study, and this difference was statistically significant.

Conclusion:  Japanese-speaking hearing aid users preferred using hearing aids that were fitted with the VAC-J strategy. Our results show that the VAC strategy can be adjusted to the frequency profile of different languages and that participants expressed their subjective preference more clearly than was reflected in the SSQ scores. A similar language-specific strategy may improve user's satisfaction while using hearing devices, and this concept may be extended to implantable hearing devices.

Not unexpectedly, this report was criticized for lack of scientific rigor. However, it began a formal awareness on the part of the consumer that simply purchasing a hearing aid based on manufacturer-advertised features did not guarantee an optimum outcome.

Sadly, such measures are the exception not the rule. For example, one year after the Consumer Reports findings were published Mueller and Picou2 found that 40% of their 110 survey respondents, which included both hearing instruments specialists and audiologists, were self-reporting routine use real ear measures to validate hearing aid programming. In a subsequent discussion of these data, Mueller3 noted that the actual number of survey respondents using real ear measures was more likely in the range of 30%. More recently, Leavitt, Clark, and Rector4 found that only 30% of their 33 survey respondents, all of whom were Doctors of Audiology, reported using real ear verification consistently with every hearing aid fitting.

While it cannot be shown that the hearing aid programming errors reported by Consumer Reports were due to lack of real-ear measures, the similarities among the data of Consumer Reports and the studies noted above are noteworthy. All of these studies suggest that, without real ear verification of hearing aid output, programming errors occur relative to well-established targets approximately 70% of the time. ff782bc1db
