"I got my card when I had an emergency with one of my horses. I had a vet bill that was rather large and I used my card to help pay that bill. Just today, I have put my card to use again. All immunization data comes from the IDPH I-CARE system using the First Name, Last Name, Date of Birth, and address provided during registration. You will not be able to access your immunization data via the Vax Verify portal if your healthcare provider submitted different information than you.
