Physicals & Registration Details

If your student plans to participate in Fall Soccer, registration must be completed through the Arbiter system (formerly Family ID) by August 11.

PHYSICALS

• Physicals must be dated on, or after, May 1, 2025 per PIAA regulations--no exceptions.

• Only the Section 6: Certification of Authorized Medical Examiner form will be accepted. You can upload the form by scanning it or taking a clear photo with your phone/tablet.

• Do not upload the Section 5 form (or any other form). If your child sees a doctor other than their primary physician, you must bring the Section 5: Health History form with you to the appointment.

Options for Completing a Physical

1. Primary Care Physician
   Schedule directly with your child’s doctor.

2. Team Doctor: Dr. David Baxter

4. Local Urgent Care Clinics
   MedExpress, CVS Minute Clinic, etc.
