HEALTH REQUIREMENTS

KINDERGARTEN   --  Physical and dental exams are required and forms are to be completed by health care providers.  Forms can be e-mailed to us or brought in to school with the student.  

3rd and 7th GRADES --  Dental exam is required and form is to be completed and returned to school.  There  is also a vaccine requirement for 7th grade.  Tdap and MCV4 vaccines are needed prior to the start of 7th grade.

6th GRADE --  Physical is required and form is to completed and returned to school.   

S-865 FORM -- Needs to be completed each school year to update our contact and medical information for your children.  You can also give permission for tylenol and/or advil to be administered if needed while at school.  This can be completed online here:  www.philasd.org/studenthealth/#forms 

MED-1 -- A Med-1 form is required to be completed by your health care provider for any medications (other than tylenol or advil)  that need to be given in school.  Medication needs to be brought in its original package with the student's name on the label.
