The Worcester Division of Public Health, Fallon Health and UMass Memorial Health Care are asking community members to participate in the 2018 Greater Worcester Community Health Assessment Survey. A CHA Survey is conducted every three years to help us understand key issues that affect our health and also assesses the strengths of our community.
The last CHA Survey was completed in 2015 and, it has played a vital role in addressing health needs in our city and region. In this process, your voice and input are important.
If you haven’t yet had the chance to complete the survey, please click one of the links, below, to start the survey so that we are able to get a well-rounded response from those who live, work and/or play in the community.
