-Worksite Vetting Forms are due no later than May 1, 2026.
-Only electronic vetting forms submitted via the following link will be accepted. Paper forms will not be accepted.
-Use this link to access the electronic form: Worksite Vetting Form.
-Worksite Vetting Forms are to be used by GDYT partners to vet all GDYT worksites where youth will be placed for a summer work experience.
-A Worksite Vetting Form must be submitted by GDYT partners after vetting each proposed worksite location.
-The Worksite Vetting Form must be completed in its entirety. Do not leave any fields blank. Be sure to place N/A in any fields that do not apply.
-The Worksite Vetting Form will provide the necessary information needed to email the required Non-Financial Worksite Agreement (NFWA) directly to each worksite.
-If Connect Detroit does not receive an electronic vetting form for a worksite, they will not receive a NFWA and therefore cannot serve as a worksite.
