FVLML Operating Procedures
FVL Medical Equipment Loan Locker
Operating Procedures
(920) 585-7072
Location: 312 W. Northland Ave. Appleton, WI
Hours of Operation: Wednesdays 4:30-6:30
Saturdays 9-11
Procedures for procuring medical equipment:
This equipment is lent out to persons in need with only a minimal amount of paperwork.
How it works: People simply show up during the hours we are open and check out the equipment they need for as long as they need it.
When they are through with the equipment, they return it to our warehouse where it will be repaired, if necessary, and reissued.
There is no charge for this equipment because it is a loan, but please consider leaving a donation. Donations help to replace worn equipment and fund community projects.
All electric equipment; wheelchairs, lift chairs, scooters, hoyer lifts and sit to stands require a note from your doctor and have a refundable $50 deposit upon the return of the equipment to the medical locker. Knee scooter deposits are $20.
All customers please remain in the waiting area of the building until they are escorted into the supply area by a customer guide of the FVLML.
Procedures for Donating or Returning loaned equipment:
Donations or returns may be dropped off during our operating hours (Wednesday 4:30-6:30 or Saturday 9-11).
Donations or Returns can be placed in the shed at the back of our building during non-business hours.
No need to return paperwork when returning loaned equipment.
We cannot pick-up equipment you borrowed from us. You promised to return those items to our building when you checked them out.
We DO NOT have the personnel resources to meet people at other times of the week, so if you need something, please come see us during our operating hours on Wednesdays or Saturdays.
