Submitting CV-19 Vaccines

To submit proof of CV-19 vaccination for students, please follow the link below:

FUTURO STUDENT CV-19 VACCINE SUBMISSION FORM

In order to modify any quarantines, a vaccine document must be submitted and verified by a Futuro staff member in advance of an incident or quarantine. Vaccination only modifies quarantines for persons without symptoms and who are not currently testing positive.

Para modificar cuarentenas, un estudinate necesita haber sumitido documentacion que ha sido verificada por un personal de Futuro Academy antes de un incidente o cuarentena. Vacunacion solo modifica cuarentenas para personas sin sintomas y que no tienen prueba positiva.
