OBJECTIVE The quest to improve the safety and accuracy and decrease the invasiveness of pedicle screw placement in spine surgery has led to a markedly increased interest in robotic technology. The SpineAssist from Mazor is one of the most widely distributed robotic systems. The aim of this study was to compare the accuracy of robot-guided and conventional freehand fluoroscopy-guided pedicle screw placement in thoracolumbar surgery. METHODS This study is a retrospective series of 169 patients (83 women [49%]) who underwent placement of pedicle screw instrumentation from 2007 to 2015 in 2 reference centers. Pathological entities included degenerative disorders, tumors, and traumatic cases. In the robot-assisted cohort (98 patients, 439 screws), pedicle screws were inserted with robotic assistance. In the freehand fluoroscopy-guided cohort (71 patients, 441 screws), screws were inserted using anatomical landmarks and lateral fluoroscopic guidance. Patients treated before 2009 were included in the fluoroscopy cohort, whereas those treated since mid-2009 (when the robot was acquired) were included in the robot cohort. Since then, the decision to operate using robotic assistance or conventional freehand technique has been based on surgeon preference and logistics. The accuracy of screw placement was assessed based on the Gertzbein-Robbins scale by a neuroradiologist blinded to treatment group. The radiological slice with the largest visible deviation from the pedicle was chosen for grading. A pedicle breach of 2 mm or less was deemed acceptable (Grades A and B) while deviations greater than 2 mm (Grades C, D, and E) were classified as misplacements. RESULTS In the robot-assisted cohort, a perfect trajectory (Grade A) was observed for 366 screws (83.4%). The remaining screws were Grades B (n = 44 [10%]), C (n = 15 [3.4%]), D (n = 8 [1.8%]), and E (n = 6 [1.4%]). In the fluoroscopy-guided group, a completely intrapedicular course graded as A was found in 76% (n = 335). The remaining screws were Grades B (n = 57 [12.9%]), C (n = 29 [6.6%]), D (n = 12 [2.7%]), and E (n = 8 [1.8%]). The proportion of non-misplaced screws (corresponding to Gertzbein-Robbins Grades A and B) was higher in the robot-assisted group (93.4%) than the freehand fluoroscopy group (88.9%) (p = 0.005). CONCLUSIONS The authors' retrospective case review found that robot-guided pedicle screw placement is a safe, useful, and potentially more accurate alternative to the conventional freehand technique for the placement of thoracolumbar spinal instrumentation.

It would be very useful to be able to draw polygons freehand (like you can in ArcGIS Pro), instead of by adding individual vertices. This would be especially useful when creating polygons that are rounded and require many vertices to properly draw.

Please add freehand line drawing too!! I'm working with folks that are converting from ArcPad to Field Maps and they could draw freehand lines in ArcPad and now I have to tell them you need to add vertex by vertex in technology 16 years later. The users have apple pencils is there any additional functionality they can use now with the apple pencil and current Field Maps line drawing?

Having the ability to freehand draw polygons like the markup in Field Maps using a stylus but with the auto-complete function available in the webapp builder edit widget to capture geometry into polygon layers would be ideal for us. It'd also be useful to have the ability to switch between the different methods of capture.

The idea I am not sure about though is the freehand drawing conversion to polygon. This sort of thing works well in ArcPro because you can toggle between free hand, creating arcs, and tracing. On top of that, its easy to enable and disable snapping in a multitude of ways in ArcPro. When you start to build this kind of complexity into Fieldmaps, it may become overwhelming for beginning users. That said, my organization would use a free hand feature.

Can you weigh in on this idea and if its on the Field Map road map? I keep thinking Field Maps is the map app but yet it is missing some key functionality with freehand delineation of lines/polygon. Currently Survey123 with geotrace and geoshape have the freehand line/polygon capabilities, earlier versions of Collector, and ArcPad, but not Field Maps??? There is mark-up but this isn't quite the functionality for a saved feature layer. This would make such a difference in our department's use of the app for aerial surveys compared to the current functionality of Field Maps.

Hello, I have an idea to create several art peaces from thick metal wire. The cutting and prepping the wire for assembly is dirty and hard job, so I would like to separate the processes of preparation and assembly. I would like to draw my art peace (let say for example a broken umbrella) and measure the length of the lines I choose to be single peaces. The problem is that so far I had only drawn rectangular stuff (made of wooden planks). And now when I try to do any freehand line It is just a mess. Even if I make it reasonably acceptable, I cannot make a second one crossing it into the 3D space.

Do I need a special pad or I can do it with the mouse?

Use the drawfreehand function. Use this function when you want to create the ROI and set the appearance in a single command. You can specify the shape and position of the ROI interactively by drawing the ROI over an image using the mouse.

Control the freehand drawing style during interactive placement, specified as a numeric or logical 0 (false) or 1 (true). When the value is false, a single click and drag gesture completes the freehand ROI. When the value is true, multiple click and drag gestures can be combined with straight edges to make a more complex freehand ROI shape.

Waypoints appear as circular shapes on the ROI edge. You can use waypoints to reshape the ROI by clicking and dragging the waypoint with the mouse. Moving waypoints modifies the freehand-drawn region between the waypoint that you clicked and the adjacent waypoints.

I was watching a tutorial where a gentleman using an older version of Inkscape was able to draw freehand lands with the triangle in shape, therefore making the lines thin at the end of the shape and thick at the beginning (where he started drawing).

Trying to follow in Inkscape 0.91 I ran into the issue where it by default drew a thin line. I tried to make it thicker by changing the width in Path Effects but that had no effect (on that shape or any I drew after). I could change the width by increasing the width of the stroke, but then I lost my "triangle" shape (the whole freehand shape was pretty much had the same line width).

Conclusions: Ultrasound-guided freehand transperineal prostate biopsy represents a safe and accurate alternative to transrectal biopsy.
