Medical Form

Please read the Policies + Regulations and the Code of Conduct (found under the Athletics Info tab). Once you and your parent/guardian have read these 2 documents, please complete our Medical Form. The Medical Form must be completed before an athlete will be given a uniform and permitted to compete.
