Volunteer Essentials: Your "encyclopedia" to Girl Scout volunteering.
CORI/SORI Acknowledgement Form: All adult volunteers - leaders, chaperones, members - are required to complete this form to participate in certain GS activities. An approved background check is valid for three years. The adult volunteer does not need to pay for the background check.
Girl Scout Activity Form: This must be completed for "high-risk" and overnight activities; not needed for activities at GSEMA properties. Complete at least 4 weeks in advance.
Annual Girl Permission Form: Includes permission for day trips, video/photo releases, GSEMA surveys, product sale programs, release restrictions, basic health history, and emergency contact information.
Safety Activity Checkpoints: A resource for safety guidelines and standards for Girl Scouts of the United States of America (GSUSA) approved activities.
Volunteer: Girl Safety Ratios: Find out how many registered adult volunteers you need for your troop.
Troop Bank Request Form: Initial step to creating a bank account for your troop.
Annual Financial Report: Form to complete at the end of each membership year.
Troop Bank Account Payment/Credit Authorization Form: This form is required to participate in the Fall or Cookie Programs and/or receive other funds from GSEMA.
Troop Cookie Coordinator Agreement: Support the annual Girl Scout of Eastern Massachusetts (GSEMA) Cookie Program by volunteering time as a Troop Cookie Coordinator.

Camping Learning Plan for Volunteers: If your troop is interested in camping, there is training you and/or your adult volunteer(s) must have.
Overnight permission form: Troops/Service Units engaging in overnights lasting 1-2 nights must have a Health History & Permission Form for Overnights for every girl.
Property Rental for GSEMA Properties: Complete this form to rent a GSEMA property for both day or overnight use.
Archery Equipment and Range Rental: Only for use at GSEMA property.
Our Service Unit is fortunate to have trained facilitators. If you are interested in an archery rental ($50 for a 2-hour block of time) you can save significantly over the individual rate of about $25 per girl if requesting a GSEMA facilitator.
Canoe Rental Form: Only for use at GSEMA property.
