The Department has developed optional-use forms which can be used by employers to provide required notices to employees, and by employees to provide certification of their need for leave for an FMLA qualifying reason. These forms are electronically fillable PDFs and can be saved electronically. Alternatively, employers may use their own forms, if they provide the same basic notice information and require only the same basic certification information.

Certification is an optional tool provided by the FMLA for employers to use to request information to support certain FMLA-qualifying reasons for leave. An employee can provide the required information contained on a certification form in any format, such as on the letterhead of the healthcare provider, or official documentation issued by the military.

Please do not send any completed certification forms to the U.S. Department of Labor, Wage and Hour Division. Return completed certifications to the employee to provide to his or her employer.

Your employer may use the WHD prototype forms or create their own version of the forms containing the same basic information. However, an employer that requests a medical certification may request only information that relates to the serious health condition for which the current need for leave exists, and no information may be required beyond that specified in the FMLA regulations. See 29 CFR 825.306, 29 CFR 825.307, and 29 CFR 825.308.

Employers must accept a complete and sufficient certification, regardless of the format. The employer cannot reject a certification that contains all the information needed to determine if the leave is FMLA-qualifying. The employer cannot refuse:

Yes. The content of the information contained within the optional-use DOL form is still applicable, regardless of the expiration date. The expiration date on the DOL forms is related to the collection of information as required by the Office of Management and Budget (OMB), and not relevant to the content of the required information.

Use this form if you are applying for U.S. citizenship and need to request an exception to the English and civics testing requirements for naturalization because of physical or developmental disability or mental impairment.

USCIS does not want you to become the victim of an immigration scam involving fraudulent medical certifications for disability exception. When you seek a medical certification for disability exception because of a physical or developmental disability or mental impairment that prevents you from learning English and civics, please keep in mind the following:

The medical certificate is a document that serves as proof that a mariner meets the required medical and physical standards. In order to receive a Merchant Mariner Credential or Medical Certificate you will need to complete the correct applications.

As of January 24, 2014, the Coast Guard began issuing medical certificates to all mariners holding valid international endorsements, per the Final Rule titled, Implementation of the Amendments to the International Convention on Standards of Training, Certification and Watchkeeping for Seafarers, 1978 (STCW), as Amended, and Changes to National Endorsements.

The Application for Mariner Medical Certificate (CG-719K) is required for Officers and Mariners with a qualified rating. The examination and form must be completed within 12 months of application date. The medical provider completing the required examination must be a physician, physician assistant, or nurse practitioner who is licensed in the US or US Territory. Use the following steps to apply.

USCG practices, with respect to the physical and medical evaluation process, continually evolve to align with current medical standards of care and current safety standards. Although Chapters 8 through 25 of the Merchant Mariner Medical Manual, Commandant Instruction Manual (COMDTINST M16721.48) provide guidance on medical conditions that may be subject to further review, the list of conditions is not all-inclusive; other conditions may be subject to further review if they are likely to pose risk of sudden incapacitation or a debilitating complication. For the most current medical information available, please contact the National Maritime Center Medical Helpdesk, 1-888-IASKNMC (1-888-427-5662).

The Application for Merchant Mariner Medical Certificate for Entry Level Ratings (CG-719K/E) is required for entry level mariners. The examination and form must be completed within 12 months of application date. The medical provider completing the required examination must be a physician, physician assistant, or nurse practitioner who is licensed in the U.S. or a U.S. territory. Use the following steps to apply.

The Merchant Mariner Medical Manual should assist medical practitioners, the maritime industry, individual mariners, and Coast Guard personnel in evaluating a mariner applicant's physical and medical status to meet the requirements of the merchant mariner medical certificate. This Manual incorporates and consolidates prior guidance on the medical evaluation of merchant mariners contained in several Coast Guard documents. The Manual includes guidance on the medical certificate and related processes, including procedures for application, issuance, and cancellation of the medical certificate.

To obtain a medical certificate you must be examined by an FAA-designated Aviation Medical Examiner (AME). As the airman you should follow these steps to apply for and obtain your medical certificate:

At your scheduled appointment, the AME will complete your medical examination and the remainder of the FAA application form. If you meet the required medical standards, the AME will issue you a medical certificate.

If your patient or their caregiver applied online for benefits, they will provide you with printed instructions for completing your medical statement (form M-01) online. If they have been approved for Temporary Disability benefits and wish to extend their claim further, they will provide you with printed instructions for completing your medical extension (form M-03) online. These instructions provide the claimant's unique Form ID, which you will need to complete your medical certification using our online system.

These forms allow you to certify to one of the CDL categories listed below, which determines if a medical variance or a U.S. Department of Transportation (DOT) Medical Examiner's Certificate is required to be on file with the Department.

If you operate in both excepted and non-excepted interstate commerce, you must follow non-excepted interstate commerce guidelines and maintain a current medical examiners certificate with the Department.

In certain instances, the Federal Motor Carrier Safety Administration may provide an exemption to specific medical conditions. For more information, see Driver Exemptions Programs. If you have been granted a federal waiver, you must include it with your medical examiner's certificate. 152ee80cbc
