We provide urinalysis collection kits exclusively to registered healthcare providers. To ensure accurate and timely processing, all kit requests must be submitted via email.
Ordering Information
Registered providers may order male and/or female urinalysis kits for use within their practice.
Providers may also request direct-to-patient drop shipment of male or female kits.
Please note: the applicable urinalysis charge is applied at the time a drop shipment is processed.
How to Place an Order

Quantity requested
For drop shipment orders, please also include:
Patient code
Complete shipping address
