FLA STUDENT-ATHLETE CHECK LIST

ITEMS TO COMPLETE BEFORE PARTICIPATING 

IN INTERSCHOLASTIC COMPETITION:

• Complete the following documents and send to the school nurse: nurse@forestlake.org)

  • FHSAA Preparticipation Physical (EL2)

  • EKG Clearance

  • Baseline Concussion Test

• Make payment at the FLA Welcome Center ($325 - varsity, $275 - JV)

• Complete the following documents and send to the athletic director: joshua.alabata@forestlake.org) 

  • FHSAA Consent & Release Form

  • SSAA Consent & Release Form

  • FLA Player Consent Form

  • FLA Player Contract

  • FLA Parent Contract

  • FLA Athletics Uniform Policy

• Complete the following NFHS Courses and send .pdf certificates to the athletic director: joshua.alabata@forestlake.org: 

  • Concussion for Students

  • Sportsmanship

  • Heat Illness Prevention

  • Sudden Cardiac Arrest

• Parents and students join the Band App for team communication and schedules (coaches will send an invite)

• Ensure academic requirements are met during the grade check dates (please see Pilot Grade Check Policy here)
