Please review the program requirements and complete the application form to be considered for residency. 

Program Requirements

Applicants must:

• Be 25 years of age or older

• Be willing to live in a drug- and alcohol-free environment

• Be able to pass an initial drug test and alcohol screening at move-in

• Be medically stable

• Agree to follow all House Rules and Policies

• Participate in the Phase Program

• Attend required meetings

• Maintain employment or actively seek work

• Treat staff and residents with respect

• Pay the program fee of (monthly)

What to Bring

Residents should arrive with:

• Clothing

• Toiletries & hygiene items

• Medications in original bottles

• Identification (ID, SS card, insurance if applicable)

• A positive attitude and willingness to grow
  
  

Do NOT bring:

• Drugs, alcohol, weapons, or paraphernalia

• Pets

• Large furniture or appliances

• Items against house rules (these will be confiscated)
