Expense Reimbursement Form 

If you have been so kind to front some expenses for Project Graduation, thank you!  The committee wants to reimburse you.  Simply fill out this form and send back to Kelly Lamo, Student Activity Account Supervisor in the office of Farmington Board of Education located in the town hall.  Please let Meredith Bartholomay know of the expense as well.

Parent Contribution/Lawn Sign Form

The letter above was sent home to all senior parents at the end of January.  

• Each family of a senior is asked to contribute $75 to the Project Graduation event. Class dues paid to the high school DO NOT cover this event. Please know that regardless of your ability to contribute, all seniors are welcome to attend.  

• “FHS GRADUATE” optional lawn signs are $15 each and will be available for pickup at FHS in early May.  Show your River Hawk spirit for your graduating senior!  *note: the lawn signs will have a new design this year. 

Business Donation Form

Do you know someone with a business that would be willing to donate to Project Graduation?  We simply can’t host Project Graduation without support!  

• Consider making a TAX-DEDUCTIBLE donation (#06-6001614) to help us meet our fundraising goal.  Any amount is appreciated.  

• All business donors will be recognized for their contributions via social media (unless preferred to remain anonymous).  

• All donations go directly to cover expenses, including facility rental, entertainment and prizes.
