Ferric Pyrophosphate citrate (FPC) is a low-molecular-weight iron salt that can be administered via the dialysate, where it crosses the hemodialyzer membrane and enters the blood. Because it donates iron directly to transferrin, unlike the previous iron compounds, it does not require processing by macrophages and, in theory, could bypass the reticuloendothelial blockade. Adult studies have indicated that it could effectively deliver sufficient iron to replace ongoing losses and maintain Hb, leading to its approval by the FDA in 2015. Treatment with FPC did not increase ferritin, possibly reflecting that it does not increase iron stores and thus carries less risk of iron overload. It is currently being investigated as a new means of meeting the increased maintenance iron requirements of pediatric patients receiving HD. A recent pharmacokinetic and safety study of FPC via dialysate and IV in 22 HD patients ages 1–18 supports the use of FPC as a maintenance form of iron in this population. Essentially delivering FPC via dialysate or IV was very well tolerated and delivered 0.06 to 0.1 mg of iron per kilogram over a standard HD session. Clearly, future larger studies are needed, but this may become a safe and efficient way of delivering iron in pediatric dialysis patients.
