Oral Vancomycin (Syrup)

Note:

1. All Oral Syrup Vancomycin started in the ward will be supplied with 1 bottle of 220mL for 10 days of treatment.

2. Any Oral Syrup Vancomycin discharge case received from Main Block HKL, kindly check with the ward for balance in the ward and to supply patient with the remaining balance. If insufficient, to supply according to the remaining duration of treatment.

3. Please add 10ml (extra) for patient prior dispensing. 

4. Oral Syrup Vancomycin 25mg/ml is placed in Fridge A01 - aras 1 (FPDU). 

5. Use amber plastic bottle.

For example: Oral Vancomycin 125mg QID x 2 days (completed 8 days out of 10 days) 

                         = 5ml x 4 x 2 days

                         = 40ml + 10ml (extra)

                              = 50ml exact volume to be dispense to patient.
