It is essential that you follow agreed processes for monitoring continence and providing individuals with support. It is also important to look out for changes that can be a sign that something is wrong.
Some people will need to have their continence monitored closely. This may include recording bladder and bowel movements. In some cases, it will involve monitoring the type of stool produced using the Bristol Stool Chart.
If you follow the care or support plan but you are unsure about anything or something changes, you should seek support from your manager or a continence specialist.
