Sections of the form will create a prepopulated score at the end of the audit supervisors are required to total these up and enter the final score out of 27

The form allows visual inspection in both settings where hot soapy water is available and when just alcohol gel is available.
Supervisors should inspect the workforce member's hands and then watch them wash their hands and rate how well they did.
If you have concerns address these at the time and follow up with an email to both their line manager and both service delivery coordinators.
At the end of the inspection, supervisors are to total up the final score and make any comments on the form.
