Please use the link to complete the Community Learning Hub Tutoring Initiative Enrichment Provider Application to indicate your interest in participating.
Once selected as an enrichment provider for 2025-2026:
Please complete the Enrichment Provider CLH Tutoring Initiative Background Check Form. All staff and volunteers must complete this form (only selected Enrichment Providers).
If your organization is selected to participate as an Enrichment Provider for the year, you will submit your documents using the Enrichment Provider Document Submission Google Form.
Session Log and Invoice submissions:
Use the following link to submit all session log and invoices for the 2025- 2026 CLH Tutoring Initiative. You will be paid $150 for each scheduled 60-minute session.
