I, the undersigned, hereby acknowledge that my child will be participating in various activities, events, and programs organized by Evolve Kids Academy. I understand that while all reasonable precautions will be taken to ensure the safety of my child, there may be inherent risks associated with these activities.
I hereby agree to the following:
1. Assumption of Risk: I understand and accept that there may be risks involved in the activities my child will engage in at Evolve Kids Academy. These risks include but are not limited to accidents, injuries, illness, or any other unforeseen circumstances. I acknowledge that it is my responsibility to inform the academy of any relevant medical conditions or allergies my child may have.
2. Release of Liability: In consideration of my child being permitted to participate in activities provided by Evolve Kids Academy, I hereby release, discharge, and hold harmless the academy, its owners, instructors, employees, and volunteers from any and all claims, suits, damages, liabilities, costs, and expenses (including attorney fees) that may arise as a result of my child's participation, except for injuries caused by their willful misconduct or gross negligence.
3. Emergency Medical Treatment: In the event of any accident, injury, or illness during my child's participation in activities at Evolve Kids Academy, I hereby authorize the academy to obtain necessary medical treatment and/or emergency medical transportation for my child. I understand that I will be responsible for any expenses incurred as a result of such treatment.
4. Photo and Video Consent: I grant Evolve Kids Academy the right to use photographs and/or videos of my child taken during their participation in activities for promotional and educational purposes. I understand that my child's name will not be used in conjunction with these materials without my explicit written permission.
I have read and fully understand the contents of this Liability and Consent Form. I acknowledge that it is a legally binding agreement, and I voluntarily agree to its terms.

This Educational Services Contract ("Contract") is made and entered into between Evolve Kids Academy ("Academy"), and the undersigned parent/guardian ("Client") of the child named below:
Child's Name: _________________________________________________________
1. Scope of Services: The Academy agrees to provide educational services to the above-named child on a week by week basis, in accordance with the Academy's curriculum and program. The services may include but are not limited to academic instruction, physical activities, arts and crafts, and any other activities deemed suitable by the Academy.
2. Schedule and Fees: The Academy will provide educational services to the child during the following days and hours each week: Day(s): ____________________________ at
Time: ______________________________. The Client agrees to pay the Academy the agreed-upon fees for each week of educational services, as outlined in the Academy's fee schedule. Payment for each week's services is due in full prior to the start of that week.
3. Termination and Withdrawal: Either party may terminate this Contract with a one-week notice. In the event of withdrawal by the Client during a week for which fees have been paid, no refund will be provided for the remaining portion of that week.
4. Code of Conduct: The child must adhere to the Academy's code of conduct, which includes but is not limited to showing respect for instructors and peers, following instructions, and behaving in a safe and responsible manner. The Academy reserves the right to suspend or terminate its services if the child's behavior violates this code of conduct, without providing a refund for any remaining weeks.
5. Liability and Consent: The Client acknowledges and agrees to the terms outlined in the Liability and Consent Form provided by the Academy. This includes assuming the risks associated with the child's participation in activities and granting permission for emergency medical treatment, if necessary.
6. Confidentiality: The Academy will maintain the confidentiality of all personal and educational information related to the child, in accordance with applicable privacy laws and regulations.
7. Governing Law: This Contract shall be governed by and construed in accordance with the laws of Georgia, USA. Any disputes arising out of or in connection with this Contract shall be subject to the exclusive jurisdiction of the courts of Georgia, USA.
8. Entire Agreement: This Contract constitutes the entire agreement between the parties and supersedes any prior understanding or agreement, whether written or oral, relating to the subject matter herein.
