All full-time employees are eligible for medical, prescription, dental, and vision benefits on the first day of the month following completion of 30 days of employment. Generally, employees complete their enrollment during new hire orientation or during the annual open enrollment. As for ancillary coverage, ProCare Rx provides employees with long-term disability and basic life insurance, and employees may elect to add short-term disability, and supplemental life insurance to their plan. The plan year for insurance is April 1 through March 31. Employee are eligible to participate in the 401(k) plan after completing one year of service.

FSA (Flexible Spending Account) & HSA (Health Savings Account)
Full-time employees may elect an annual contribution to flexible spending accounts (FSA) OR limited flexible spending accounts (LFSA) along with a health savings accounts (HSA). These accounts are on a pre-tax basis (i.e., the contributions are deducted from gross income before tax, thereby reducing the employee's taxable income and tax burden). Health Savings Accounts are available only to employees who select the High-Deductible Health Plan. Elections must be made during new hire orientation or open enrollment; FSA and LFSA elections cannot be changed during the plan year, per IRS regulations. These accounts are established separately for medical, dental, or vision expenses not covered by insurance (e.g. deductibles, co-pays, over-the-counter medicine purchases, orthodontia, upgraded vision care) and dependent care (e.g. day care expenses from a qualified provider for children under age 12). You pay for eligible expenses by using a debit card that will be issued to you. If you make an eligible purchase and pay out of pocket, please log on to EBMS.com and go to the FSA page for instructions on reimbursement.
To check your balance, submit a claim, or view other information relating to your FSA, log on to https://mibenefits.ebms.com/auth/login and go to the Flexible Spending Account section.
If you enrolled in the FSA, Limited FSA, or the HSA, click on the "Enrollment Next Steps" to your left for instructions on accessing your account with Admin America (plan administrator). You will also notice helpful information to guide you along.

Employees may also take out a loan from their 401(k) balance and repay the loan through payroll deduction.
Who's eligible?
Full-time employees may participate in the company 401(k) plan at the beginning of the calendar quarter following one year of service. If you have completed 1-year of service with ProCare Rx, you are eligible to participate in the 401(k) plan.
Participants may contribute up to 25% of their gross income on a pre-tax basis up to a limit set annually by the IRS. The company match of the employee's contribution to the plan is currently 100% of the first 3% of the employee's contribution, and 50% of the next 2% of the employee's contribution. This match is fully vested with no waiting period after eligibility. No vesting schedule.

Eligible employees will received a notification and instructional information from Benefits For You (by TruStage) via regular US mail when their eligibility date draws near, and will also receive an email reminder from Human Resources. Once received, follow the instructions for a simple online enrollment and set up your retirement investments.
Enrollment, contribution percentages, and investment account elections are managed by the employee via Benefits For You at www.benefitsforyou.com thereafter.
