Why Elder Ready Exists
After decades in nursing, I saw the same problem again and again: families wanted to help an aging parent stay safely at home, but they were often given pieces of information without anyone helping them connect the dots.
A hospital discharge packet may be dozens of pages long. A family may be told to “use the walker,” “watch for falls,” or “install a grab bar,” but important questions remain:
Which walker? Used how? A grab bar where? Is the bathroom the real issue, or is the bigger problem weakness, dizziness, medication timing, rushing to the toilet, or difficulty getting in and out of bed?
That is where Elder Ready comes in.
Elder Ready helps families prepare for future changes and navigate the changes already happening — whether they are part of normal aging, temporary after illness or hospitalization, related to a medical condition, or likely to be permanent.
The goal is not to provide a medical assessment or replace the person’s healthcare team. My role is to help families look at the whole situation, understand what may matter most, and turn general advice into practical next steps.
Sometimes that means making sense of a long discharge plan. Sometimes it means finding an easier way to approach a transfer. Sometimes it means recognizing that “the bathroom needs grab bars” may be only one part of a larger problem.
The value is in bringing clinical experience, practical home-safety knowledge, and caregiver perspective to the same table.
Elder Ready recommendations are individualized, realistic, and focused on what can help this person, in this home, at this stage — without assuming every family needs major renovations or expensive equipment.
And because aging in place affects the whole family, Elder Ready also gives caregivers clear guidance, practical education, and help knowing what to do next.
