No Criminal Records Document (KRK)

Applicable Information
Effective February 15th, 2024: In connection with the amendment of the Act of May 13, 2016, on the prevention of threats of sexual crime (i.e., Journal of Laws of 2023, item 1304, as amended), students are required to submit documentation prior to activities related to treatment, education, or care of minors (i.e., individuals under 18 years of age) as part of their study program (clinical exercises/practical classes with patients, student internships).
This means that you will need to submit a No Criminal Records Document if you fall under the following categories:
All students admitted after May 29th, 2024, within the 1st Year
Students wishing to join specific student organizations such as the International Federation of Medical Students' Associations, as part of their requirement.
Documents to be Submitted to the Dean's Office
I. Certificate from the National Criminal Register (NCR) – Polish citizens and Foreigners - obtained in Poland - Original, Paper Version Required
II. Declaration of the country or countries where the student has resided in the last 20 years, other than the Republic of Poland and the country of citizenship, along with information from the criminal records of those countries obtained for the purposes of professional or voluntary activities related to contacts with children – Polish citizen and foreigner.
III. Information from the criminal register of the country of citizenship obtained for the purposes of professional or voluntary activities related to contacts with children along with sworn translation – foreigners, and students who resided in countries other than the Republic of Poland or the country of citizenship [Certificates in English do not require sworn translation] - Original, Paper Version Required
If the country of citizenship does not issue criminal records certificates, students must submit a signed declaration.
I. Certificate from the National Criminal Register (NCR)
You must personally go in person and obtain this document at the Lublin-Zachód District Court in Lublin (PL: Sąd Rejonowy Lublin-Zachód) located at Konrada Wallenroda 4d. You will need to prepare the following document prior to heading there:
Application Form for the Certificate
Proof of Payment for One Court Fee Stamp (30zl Value)
Documentation (Required: Passport; If you have, please also bring: Student Card, PESEL Number Documentation, Residence Card, etc.)
Depending on a case-by-case basis, you may receive this document immediately or have to wait for a few days.
II. Declaration of the Countries of Residence & Citizenship
The declaration form regarding the country or countries where the student has resided in the last 20 years, other than the Republic of Poland and the country of citizenship, along with information from the criminal records of those countries obtained for the purposes of professional or voluntary activities related to contacts with children is attached. The declaration is made under penalty of criminal liability for making a false statement.
III. Information from the Criminal Register of Country of Citizenship
1.What to do if the law of the country of citizenship or the country/countries of residence in the last 20 years, other than the Republic of Poland and the country of citizenship, does not provide for the issuance of information for professional or voluntary activities related to contacts with children?
You should submit information from the criminal register of that country/countries along with a sworn translation.
2. What to do if 1) the law of the country of citizenship, 2) the law of the country/countries of residence in the last 20 years, other than the Republic
of Poland and the country of citizenship, from which information from the criminal register is to be submitted for professional or voluntary activities
related to contacts with children, or information from the criminal register of that country, does not provide for its issuance or if there is no criminal register in that country?
