Team Roster Form (completed by Team Manger) 

5. Medical Waiver

    Player Medical Waiver Form (completed by Parent of players)

6. Insurance (If you don’t have team insurance, you may use individual player proof of insurance.)

Team Insurance (Team Manager)

Private Insurance (Parents) **Only complete if there is NO team insurance

The above items must be submitted BEFORE your team will be added to the schedule. If you have any changes to your roster after Tuesday 7/21 you must bring a HARD COPY of the signed Roster/Wavier and turn in to the concession stand PRIOR to your first game.
